- Printed or typed on pleading paper in local courts; plain paper in appellate courts.
- Pleading paper can be 28-line or 36-line (double-spaced or 1 1/2 spaced)
- Two-hole punched at top
- Twenty pound or heavier recycled paper (25%) - 8.5 x 11 inches
- One inch margins all around for appellate courts, lower courts allow narrower right margin
- Body is double-spaced, quotes, property descriptions, can be single-spaced
- 12 point typeface
- One side of paper only
- Page numbers at bottom of page with horizontal line beneath
- Title of pleading below horizontal line at bottom of page - 10 point typeface or greater
- Stapled (or bound) at top; appellate court, bound along left side (or stapled and taped)
- Name, address, phone number, bar number for attorneys - in upper left corner
- Who you represent or "In Pro Per" if you represent yourself - under address
- Name of court three inches from top - centered
- Name of case on left under name of court with party status indicated, ie., John Doe, Plaintiff vs. etc.
- Case number on right under name of court (Name of referee under case number if assigned)
- Title of pleading on right under case number
